Background

ESD (ESD) therapy-based minimally invasive treatment of digestive endoscopes has become an option, even a priority, for early stage treatment of mucosal tumors of the digestive tract and certain submucosal tumors. For digestive tract early cancer and precancerous lesion without lymph node metastasis and with shallow infiltration depth and tumors under the digestive tract mucosa, complete resection and curative resection can be achieved by using ESD. In the conventional ESD operation, the mucosa layer and the muscle layer are stripped by injecting related medicaments under the mucosa, the needle type incision knife is used for respectively pointing out the excision boundaries point by point, and then the needle type knife is used for incision and stripping by an IT knife. However, for some lesions with large area, unclear boundary and adhesion, the conventional ESD process is particularly tedious and time-consuming when treating the lesions, and is prone to bleeding, perforation, incomplete mucosal peeling, lesion residue and the like during operation, while the choice of open surgery for treatment causes problems of overlarge wound, excessive treatment and the like.

Chinese patent application CN208958311U discloses an auxiliary tube for endoscopic gastrointestinal mucosa peeling, which comprises an auxiliary tube, a control rod and at least two guiding hoses, wherein at least two through-hole pipelines are arranged on the tube wall of the auxiliary tube along the axial direction of the auxiliary tube, one end of each guiding hose is communicated with each through-hole pipeline, the through-hole pipelines and the guiding hoses corresponding to the through-hole pipelines form a control rod channel, and the control rod is movably arranged in one of the control rod channels in a penetrating manner. The hemostatic clamp can be not limited by an operation part, the directions and the forces of traction and pushing can be adjusted in real time, the control hand feeling of an endoscope cannot be influenced, and the mucosa clamped by the hemostatic clamp is pulled through the traction rope on the rod, so that the whole mucosa is stripped, the specimen is still tied to the traction rod, the detachment of the specimen is not needed to be worried after the operation, and the wound surface is directly treated. It only addresses the problem of target pull-back, which does not accomplish systematic optimization of existing ESD operation.

In summary, there is a need for a novel endoscopic mucosal dissection device that can simplify ESD operation steps, achieve simple and convenient disposable excision under an endoscope and complete recovery of excision focus, and reduce lesion residues.

Disclosure of Invention

The invention aims to provide a novel endoscopic mucosal dissection device which can simplify ESD operation steps, realize simple and convenient one-time excision under an endoscope and complete recovery of an excision focus and reduce lesion residues.

In order to achieve the purpose, the invention adopts the technical scheme that:

a novel mucosa peeling device under an endoscope comprises a main pipe body, an injection module, a cutting module and a traction module, wherein the injection module, the cutting module and the traction module are movably connected with the main pipe body; the injection module comprises a plurality of injectors for injecting the target object; the cutting module comprises a plurality of cutters for cutting the target object; the traction module is used for fixing a target object; the tail ends of the cutting modules form a net-shaped structure.

As a preferred technical scheme, the traction module is located at the center of the main pipe body, the injection module and the cutting module are uniformly arranged around the main pipe body in the axial direction, and the cutting module is arranged outside the injection module.

Referring to fig. 1, fig. 1 is a perspective view of a novel endoscopic mucosal dissection device according to the present invention. A novel mucosa stripping device under endoscope comprises a main pipe body 1, an injection module 2, a cutting module 3 and a traction module 4; the main pipe body 1 is of a tubular structure, a plurality of channels are arranged in the main pipe body and used for accommodating the injection module 2, the cutting module 3 and the traction module 4; the injection module 2 comprises a plurality of injectors, the injectors are tubular structures with injection needle tubes, and can inject liquid into a target object, namely an object to be cut, such as a mucous membrane and the like, in the embodiment, four groups of injectors are adopted, and as understood, different groups of injectors, such as 2 groups, 3 groups, 6 groups and the like, can be selected according to actual needs, and in some preferred embodiments, the injectors are detachably connected with the main tube body 1 through channels thereof and can be flexibly configured according to needs; the injector of the injection module 2 is movably connected with the main pipe body 1 and is uniformly arranged around the main pipe body 1 in the axial direction; the cutting module 3 comprises a plurality of cutters for cutting the target object, in this embodiment, the cutters are electrode guide wires, and are provided with 4 groups, are uniformly arranged around the main pipe body 1 in the axial direction and are arranged outside the injection module 2, and in some embodiments, the cutters can also be provided as 2 groups, 5 groups, 6 groups and the like; the cutter of the cutting module 3 is movably connected with the main pipe body 1, the cutter is an electrode catheter, the tail end of the cutter is an exposed electrode guide wire structure, power can be supplied by an external power supply, and a reticular structure is formed between the cutters; the traction module 4 is arranged at the center of the main pipe body 1, is movably connected with the main pipe body 1, is a negative pressure suction pipe, has a diameter larger than that of the pipe body at the tail end thereof in some embodiments, is in a bowl-shaped inverted buckle shape, and forms a negative pressure traction effect on a target object through an external negative pressure source.

Example 2

Referring to fig. 3, fig. 3 is a schematic view of another novel endoscopic mucosal dissection device of the present invention. The present embodiment is substantially the same as embodiment 1, except that the main tube 1 further has a camera 11 and a lighting device 12 at its end, and the camera 11 and the lighting device 12 can have multiple sets, so as to provide a multi-angle view during the operation, thereby avoiding excessive adjustment of the main tube 1 and the suction fixing position.

Referring to fig. 4, fig. 4 is a schematic view of a surgical instrument according to the present invention. A surgical instrument is characterized by comprising a novel endoscopic mucosa stripping device, a control system 5 and a connecting system 6, wherein the control system 5 is connected with the novel endoscopic mucosa stripping device through the connecting system 6; the control system 5 is used for providing a high-frequency electric switching power supply, a traction source, namely a negative pressure suction source and the like; the surgical instrument can provide stable working environment, energy source, operation foundation and stable control for the novel endoscopic mucosa stripping device.

The surgical instrument comprises a novel endoscopic mucous membrane stripping device, and can determine the boundary and range of mucous membrane stripping in the use process, push out an injector through a channel of a main pipe body 1, observe under an endoscope, adjust the position according to the size of a lesion and carry out multi-point submucosal injection, attract and fix the lesion mucous membrane by a negative pressure suction tube after the lesion of the mucous membrane is uplifted by injection, push out an annular electrode guide wire of a cutting module 3, and simultaneously carry out power-on cutting along the fixed range and close to the excision edge of the mucous membrane, so that the lesion of the mucous membrane is completely excised and stripped at one time, and then shrink the whole reticular tail end to recover the complete lesion; the operation steps of ESD are simplified, the excision and the complete recovery of the excision focus are simply and conveniently completed at one time under an endoscope, and the pathological change residue is reduced.
