Background

The colon is not emptied smoothly after the stoma is narrowed, which is easy to cause the frequent defecation or the thin excrement, and the clinical manifestation of low-level incomplete obstruction can also appear. The causes of stoma stenosis are mainly as follows: (1) colostomy stenosis is mostly due to ischemia. (2) Topical infections and skin openings too small. (3) The exposed serosa of colon is stimulated by feces, etc. to cause serositis, resulting in granulation tissue hyperplasia, followed by scar contraction, forming a ring-shaped stenosis.

The current treatment based on stoma stenosis is as follows: (1) patients with less stenotic stomas can be irrigated with a funnel or catheter. (2) When the narrow ring can pass through all the little fingers or the tips of the index fingers, the stoma is dilated by the fingers 1-2 times every day, and the degree of passing through all the index fingers is determined. Sufficient dilation often causes severe pain, easily producing bleeding and more scar tissue. (3) For symptomatic patients who are not effective in non-surgical treatment, or when the narrow ring cannot pass through the little finger, a ring of scar tissue on the periphery of the ostomy intestinal segment is removed circularly, the intestinal wall and the skin edge are sutured back intermittently by a thin intestinal thread, or the incisal edge is sutured back by a radial incision and a Z-shaped incision.
