Technical Field

Background

Defects in the alimentary tract, particularly perforations, are a common problem encountered during endoscopic procedures. At present, the treatment modes under the endoscope comprise titanium clip closed suture, nylon rope and titanium clip purse-string suture and some suture devices reported abroad. The first two methods are the most common methods in China and have certain limitations. Due to the limited angle of opening of the titanium clip, it is not effective for large perforations. In addition, delayed perforation with congestion and swelling of mucosa at the perforation edge also has difficulty in sealing the perforation wound by a titanium clip. The nylon rope combined with the titanium clip is sewn in a purse-string manner, so that the large-size perforation can be repaired, but the flexibility is poor.

Detailed Description

The present invention will be further described with reference to fig. 1 to 18.

The invention relates to a stitching instrument, which comprises a main body part 11, a hand-held part and a working part, wherein the hand-held part and the working part are respectively arranged at two ends of the main body part 11, the working part penetrates stitching nails through tissues to be stitched and bends the tips of the stitching nails, and the shapes of the stitching nails are similar to the shapes of stitching nails used by a stitching machine.

The working part comprises an anvil 12, a staple plate 13 for mounting staples and a mounting part 14, the staple plate 13 is arranged on the mounting part 14 in an articulated manner, the anvil 12 is arranged on the mounting part 14, and a space area between the staple plate 13 and the anvil 12 is a space area where tissues to be sutured are located.

In the process of using the suturing device, tissue to be sutured is clamped and pulled into a space area between the nail plate and the nail anvil by a wide-angle endoscope live clamp or an endoscope foreign body clamp or an endoscope three-leg clamp, and then suturing is carried out.

The handheld portion comprises a handheld handle 15 and a movable handle, the handheld handle 15 is mounted on the main body portion 11, the movable handle is mounted on the handheld handle 15 in a hinged mode, the movable handle enables the nail plate 13 to swing through the traction portion 16, and the main body portion 11 is formed with a first through hole through which the traction portion 16 can pass. The traction portion 16 is made of a metal material having both hardness and elasticity. The articulated shaft of the movable handle and the hand-held handle 15 is provided with a torsional spring, and the nail plate 13 is convenient to reset after being sewed through the action of the torsional spring.

The movable handle comprises a third connecting part 34 for connecting the traction part 16 and a holding part 35 for holding, and the third connecting part 34 and the holding part 35 form a V-shaped structure. The grip portion 35 is formed with a wave-shaped structure to increase friction force during operation.

The movable handle takes a hinged point as a fulcrum to pull the traction part 16, the traction part 16 drives the nail plate 13 to collide with the nail anvil 12, the suturing nail pierces the tissue to be sutured, the tip of the suturing nail is bent, the nail plate 13 resets, and the suturing nail is separated from the nail plate 13.

The anvil 12 has a working surface formed with working slots corresponding to the staples, the working slots including a transverse slot 23 and vertical slots 24 at both ends of the transverse slot 23. After the staples pierce the tissue to be stapled, the two tips of the staples respectively abut against the bottom walls of the two vertical slots 24, the nail plate 13 continues to swing towards the anvil 12, the tips of the staples move along the vertical slots 24, the two tips enter the transverse slots 23, and then the two tips of the staples are bent oppositely under the impact of the nail plate 13 towards the anvil 12.

the stitching instrument further comprises an operating mirror 38, the mounting part 14 is arranged at the end part of the operating mirror 38, and the mounting part 14 is a sleeve structure sleeved at the end part of the operating mirror 38. The main body part 11 is arranged on the side surface of the operating mirror 38 through at least one fastener 39, the fastener 39 is provided with a fifth through hole for the main body part 11 to pass through, the nail plate 13 is provided with a notch corresponding to the operating mirror 38, and the notch is designed to avoid the nail plate 13 to shield an operating field when swinging towards the nail anvil 12.

the mounting portion 14 is formed with a first mounting structure 17, the nail plate 13 being arranged with a first articulated portion 18, the first articulated portion 18 being mounted with the first mounting structure 17 by means of a first articulated shaft 19, the nail plate 13 being swung about the first articulated shaft 19. The first mounting structure 17 is formed with two first protruding ends with a first hinge 18 located therebetween, and a first hinge shaft 19 passing through the two first protruding ends and the first hinge 18.

The first hinge portion 18 is provided with a second connecting shaft 20, the second connecting shaft 20 is deviated from the position of the first hinge shaft 19, and the pulling portion 16 applies force to the nail plate 13 through the second connecting shaft 20, thereby swinging the nail plate 13.

The second connecting portions 21 are respectively arranged at two ends of the second connecting shaft 20, two fixing portions 22 are formed at one end of the traction portion 16, the fixing portions 22 are correspondingly mounted on the second connecting portions 21, the fixing portions 22 are located at one ends of the second connecting portions 21, and the mounting positions of the fixing portions 22 and the positions of the nail plates 13 are located on the same side. This can be regarded as the first embodiment of the working part.

The ends of the two second connecting portions 21 remote from the second connecting shaft 20 are connected to each other, i.e. the two second connecting portions 21 form an annular structure in which the mounting portion 14 is located. One end of the traction part 16 is formed with one or more fixing parts 22 for fixing with the second connecting part 21, and the mounting position of the fixing part 22 is opposite to the position of the nail plate 13. This can be regarded as embodiment two of the working part.

The nail plate 13 is formed with a mounting groove 25 for mounting the suturing nail, end walls are respectively formed at two ends of the mounting groove 25, the end walls of the mounting groove 25 can prevent the position deviation of the suturing nail, and the dislocation of the suturing nail and the working groove is avoided.

The side walls of the mounting grooves 25 are provided with stoppers 26, the stoppers 26 have stopper protrusions 27, staples are loaded into the mounting grooves 25, and the staples are prevented from coming out of the mounting grooves 25 by the stoppers 26.

The stopper 26 includes a first portion 28, a second portion 29, and a third portion, the first portion 28 and the second portion 29 being arranged oppositely, the first portion 28 being mounted with the second portion 29 through the third portion, and the stopper projection 27 being located at one side of the second portion 29. The retainer 26 has a U-shaped configuration, and the retainer 26 has elasticity so that when staples are loaded, the staples are pushed out of the area between the retaining protrusions 27 and the side walls of the mounting grooves 25. The both sides of spacing arch 27 are shaped respectively has the guide face of slope, conveniently sews up the nail and fills to mounting groove 25, conveniently sews up the nail and breaks away from mounting groove 25.

The nail plate 13 is formed with a containing groove 40 for placing the stopper 26, a fourth through hole communicated with the mounting groove 25 is formed on the side wall of the containing groove 40, the limiting protrusion 27 penetrates through the fourth through hole, and the side surface of the second part 29 is abutted against the inner side wall of the containing groove 40.

The hinged joint of the movable handle and the hand-held handle 15 is provided with a locking nut, and the nail plate 13 and the anvil 12 are locked in a proper closed state through the locking nut, which can be regarded as an embodiment two of the hand-held part.

A rotatable positioning support 32 is arranged on the movable handle, and a positioning groove 33 corresponding to the positioning support 32 is formed on the handheld handle 15. The end of the positioning support 32 bears against the positioning slot 33, keeping the plate 13 and the anvil 12 properly closed. It is also contemplated that the movable handle may be formed with a detent 33 and the handle 15 may have a detent support 32 disposed thereon. This can be considered as embodiment three of the handle 15.

The main body 11 is formed with a second through hole, the second through hole is arranged with a lead 36 for conducting high-frequency electricity to the staple, the side of the handle 15 is formed with a third through hole 37 for the lead 36 to pass through, and the lead 36 is connected with an external device after passing through the third through hole 37. In the working part, in order to prevent the wire 36 from interfering with the suturing operation, in the first embodiment of the working part, the wire 36 is routed along the side of the nail plate 13, and in the second embodiment of the working part, the wire 36 is routed along the second connecting portion 21 and then along the side of the nail plate 13.

If the tissue to be sutured is thick and not easy to puncture, a perforation can be cut on the tissue to be sutured in a high-frequency cutting mode, and the suturing nail passes through the perforation.
